In this November 2021 issue, we will again discuss the topic of\u00a0<\/span>#Fulldigitalworkflow<\/span><\/em><\/strong>. <\/span><\/p>

Thanks to digital technologies, we will show you how a simple job can\u00a0<\/span>be completed<\/span> using examples from our daily practice.\u00a0<\/span><\/p>

This<\/span>\u00a0is the prosthetic completion of a surgical work we presented at DentalTech last May. It is entitled: \u201cGuided surgery: a technology for everyone?\u201d <\/span><\/p>

This time, we will focus on prosthetic restoration, our main objective since the patient asked us to do this. <\/span><\/p>

With proper preparation and time spent on good training, everyone could easily apply digital dentistry daily, making it easier to treat simple and complex cases. <\/span><\/p>

In the last issue of DentalTech in May, we presented a clinical case of implants placed in the mandible through a new method (Hypnoguide) with customized and laser-sintered titanium templates.<\/span><\/p>

As promised, we present the prosthetic development of the same case, with the entire workflow from the intra-oral scan to the delivery of provisional and definitive restorations.\u00a0<\/span><\/p>

This<\/span>\u00a0is the finalization of the case in the mandible; in one of the following issues, we will present the implant and prosthetic rehabilitation carried out in the same patient in the upper jaw. <\/span><\/p>

In this case, we opted for rehabilitation with monolithic restorations in translucent zirconia on both the natural teeth and the implants. The approach followed was that of\u00a0<\/span>#Fulldigitalworkflow<\/span><\/em><\/strong>\u00a0and the\u00a0<\/span>#Modelfree\u00a0<\/span><\/em><\/strong>protocol.<\/span><\/p>

Of course, this approach is only one of many: 3D printing is often indicated and is a valuable aid for dental technicians: physical models\u00a0<\/span>are\u00a0still used<\/span>\u00a0today. <\/span><\/p>

But this does not detract from the fact that it\u00a0is now possible to proceed in a<\/span>\u00a0fully integrated digital\u00a0way<\/span><\/strong>.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

The present case represents the prosthetic development of what was presented in the May 2021 issue of DentalTech.<\/p>

Two months after implant placement\u00a0<\/span>(Figs. 1, 2<\/span><\/strong>), the patient underwent an intraoral CT scan (CS 3700\u00ae, Carestream Dental, USA). The scan for the implants included first the capture of the antagonist arch, then the master arch (where the fixtures\u00a0<\/span>are located<\/span>) and the mucosal collars, after the removal of the gauze abutments, and finally the capture of the bite (occlusion record), both on the right and on the left.<\/span><\/p>

After the healing abutments are removed, an impression of the mouthguard (occlusal register) is taken on both the right and left sides.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

After careful verification of the goodness of the occlusion register,\u00a0<\/span>the area of the mucosal collars was highlighted<\/span>, and the scanbodies were screwed in. In this case, the implants (Anyridge\u00ae, Megagen, South Korea) provided for the screwing of 13 mm high scanbodies.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

It is essential to ensure that the scanbodies\u2019 position and the hexagons\u2019 coupling are correct. If there is any doubt, it is best to check by X-ray that the CT abutments\u00a0<\/span>are correctly inserted<\/span>\u00a0into the fixtures<\/span><\/strong>.\u00a0 The STL files derived from the intraoral scan\u00a0<\/span>are sent<\/span>\u00a0to the dental technician who models in CAD (Valletta\u00ae, Exocad, Germany) a provisional and preliminary dental restoration to\u00a0<\/span>be adapted<\/span>\u00a0to the natural stumps of the frontal group and the individual definitive stumps to\u00a0<\/span>be screwed<\/span>\u00a0onto the implants, with the corresponding provisional restoration (<\/span>picture 3)<\/span><\/strong>.
Using individual hybrid abutments consisting of a company-supplied titanium Tibase, onto which an individual CAD-modelled and milled zirconia abutment\u00a0is placed, has clear advantages over the conventional alternative of a screw-retained superstructure.

Firstly, the interface with the soft tissues is managed through zirconia from the very first provisionalization: this allows better tissue healing with the formation of an adequate biological seal, compared to what can be achieved when resins or PMMA are used in the emergence. Secondly, the individual hybrid titanium\/zirconia abutment allows the management of the emergence anatomically better than modeling a screw-retained superstructure in terms of volumes. The third and last aspect that should not be overlooked is that the individual hybrid abutment is permanently screwed onto the implant and is never removed. This prevents damage to the soft-tissue bond formed when the temporary restoration is replaced by the permanent one.<\/p>

This does not occur in the case of a screw-retained superstructure, where the removal of the temporary and its replacement with the permanent determines the inevitable loss of any mucosal seal. These advantages are essential and are an excellent reason to go down a technically more challenging route, such as the individual abutment, which represents an extra interface to be managed in CAD. Maintaining rigorous precision with strict phase control is necessary to ensure an excellent clinical outcome in the medium and long term.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

After milling the temporary PMMA dental restorations and zirconium oxide hybrid abutments on a powerful 5-axis milling machine (<\/span>DWX-52D Plus<\/a><\/span><\/span>, DGSHAPE, a Roland DG Group Company, Japan<\/span><\/strong>), the abutments are sintered and cemented extra orally on a titanium bonding base in the laboratory. The dental restorations are then ready for delivery.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

During the second appointment at the practice, we apply the temporary and preliminary dental restoration to the anterior group.<\/p>

The latter\u00a0<\/span>is correctly modeled<\/span>\u00a0in CAD following the preparation of the teeth and the screwing of the various hybrid abutments onto the implants, with the cementing of the temporary teeth on top<\/span>\u00a0(Photo 4)<\/span><\/strong>.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

At the same time, the final impression is made using the double wire technique to clearly show the margins of the prosthetic preparation, always using the same intraoral scanner.<\/p>

This impression is also captured with the provisional teeth on already cemented implants to give a definite occlusal reference<\/span><\/strong>\u00a0based on the initial intraoral scan study\u00a0cases involving discrete chewing enhancement (in this case, the patient requested that the upper arch\u00a0<\/span>be treated<\/span>\u00a0at a later date, after completing the total rehabilitation of the lower arch)\u00a0<\/span>(Photo 5<\/span><\/strong>). It is essential at this stage to use all available tools\u00a0with the intraoral scanner.<\/span><\/p>

A scanned image was also captured and aligned with the previous one, with the anterior provisional tooth restoration\u00a0<\/span>in situ<\/span><\/em>\u00a0already fitted and functionalized (<\/span>Photo 6<\/span><\/strong>). This scan is helpful for the dental technician to have anatomical and functional limits with which to model the anatomy of future permanent dental restorations. The dental technician can then model the final dental restorations\u00a0on a natural stump, with simple monolithic crowns made of translucent zirconia (<\/span>Photo 7<\/span><\/strong>). At the same time, it models the final dental restorations on the implants without the need for further scans.<\/span><\/p>

One of the\u00a0<\/span>great<\/span>\u00a0advantages of CAD is the ability to check the thickness of monolithic dental restorations in 3D (<\/span>Photo 8<\/span><\/strong>).<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

As the protocol calls for a model-free process, it was considered appropriate to check the marginal closures, approximal fit, and occlusal contacts of the shapes of future final dental restorations using 5-axis milled replicas.<\/p>

The CAM software used for milling (Millbox\u00ae, CIMsystem, Italy) (<\/span>Photos 9,10<\/span><\/strong>) was always the\u00a0same.\u00a0<\/span>It\u00a0allowed the work to\u00a0<\/span>be carried out<\/span>\u00a0predictably and\u00a0<\/span>simply<\/span>, guaranteeing absolute quality.<\/span>\u00a0Thanks to<\/span>\u00a0this software<\/span>, it was<\/span>\u00a0possible to check the marginal closures and the quality of the interproximal and occlusal contacts.<\/span>\u00a0It was also\u00a0<\/span>possible<\/span>\u00a0to make a monolithic translucent zirconia finish.

Cementing\u00a0was carried out\u00a0in detail, and the procedure was completed\u00a0(Photo 11)<\/strong>\u00a0with the patient and the operator fully satisfied.<\/strong>
